Medical Examination Application Form

Application Process

  1. Fill up the Applicant’s Information and choose the type of application.
  2. Complete the required details and requirements (2 MB file size max.) and submit accordingly.
  3. You will receive a reference number after completing the request and keep it for important purposes.
  4. Upon successful submission of your application, our representative will review your complete documents. (in case some additional documents or information are needed you will be notified by email) if completed, we will process your application.
  5. A payment link will be sent to your email or SMS (for payment options, we have the option for bank transfer, credit card, and cash deposit) to further your application.
  6. Once the transaction is completed, you will be notified through SMS and receive the application form directly from (ICA)- Federal Authority for Identity and Citizenship Authority.
